What it involves
A session typically includes an induction — guiding attention inward and encouraging physical stillness — followed by suggestions: that you will feel sleepy at the right time, that you can let go of effort, that the bed is associated with rest. Many practitioners provide a recording for home use, since nightly repetition is where most of the effect would come from.
What the evidence says
Trials of hypnosis for insomnia are generally small and of variable quality. Meta-analytic evidence suggests moderate improvements in sleep onset and quality, but the literature is thin compared with CBT-I. Hypnosis is sometimes combined with CBT-I, and the combination looks reasonable.
The honest position: it is not a first-line treatment, it appears safe, and some people find it helpful where other approaches have not landed.
Who it tends to suit
- People who respond well to guided imagery and suggestion.
- People whose insomnia has a strong performance-anxiety component — the suggestion "you do not need to try" maps directly onto that problem.
- People who find purely physical techniques unsatisfying.
People who are sceptical of the framing tend not to benefit, which is unsurprising.
Cautions
- Seek a properly trained, registered clinical hypnotherapist.
- Hypnosis is not appropriate as a sole treatment where there are underlying conditions — apnoea, restless legs, mood disorder — that need assessment.
- Avoid practitioners who make claims about curing organic disease.
- Recordings are generally safe, but do not use anything that requires active attention while driving or operating machinery.
Alternatives with similar logic
Much of what makes hypnosis work — guided attention, reduced effort, and a suggestion that you do not need to try — is available elsewhere:
- Body scan — guided attention without suggestion.
- Yoga nidra — guided lying-down practice with a similar structure.
- Paradoxical intention — removes the performance goal directly.
- Feldenkrais movement — for when the resistance is physical rather than mental.
